For keeping up with screenings and checkups

Know which checkups you've had, and when.

The trouble with dates

The dates live in other people's files.

Your last colonoscopy is in a gastroenterology practice's system. The mammogram sits with an imaging center. Your cholesterol was checked by a doctor you saw before you moved. Each one keeps its own piece, and none of them keeps the list.

So when a form asks for the date of your last Pap test, you guess. When your doctor asks whether you're up to date, you aren't sure. That isn't carelessness. Nobody ever handed you the whole list.

Illustrative example

Your screening history, in one answer

One question pulls the dates and findings out of reports from different places. What to do next is a conversation with your doctor, and now you'll walk in with the facts.

You

When did I last have each of my screenings?

Perennial

Here's what I found in your records.

ColonoscopyAug 2017Two small polyps removed, both benign. The report suggested a repeat in 5 years.
MammogramFeb 2025Read as normal.
Pap and HPV testJun 2022Both negative.
Cholesterol panelJan 2026LDL 128 mg/dL, up from 112 in 2024.
Flu shotNot foundNothing in your file. If you had one at a pharmacy, you can upload the record.

Your doctor can tell you which of these are due for you. Would you like a reminder to ask?

You

Remind me the first Monday in January to book my physical.

Perennial

How it works

Three steps to the whole list

What setting up asks of you, in order.

  1. Start your trial

Start your 7-day free trial and choose who the profile is for: you, or a parent you look after. Tell Perennial where you live and choose to connect your records.

  2. Verify, consent, meet a clinician

Confirm your identity, sign the consent forms and book an intake visit with a clinician, who signs your records request with you.

  3. Gather the dates, then ask

Triangle requests your records through the national health-record networks, and you upload anything else, like a pharmacy vaccine record. Then ask when you last had each test, and set reminders in plain words.

What you get

Your checkups, kept straight

Perennial works from your own records, so the answers are about you, with the dates and the findings in one place.

  • Your checkup history, from your records

Ask "when was my last Pap test?" or "what did my colonoscopy find?" and get the answer from your own records, in plain language.

  • Results you can follow over the years

Cholesterol, A1c and other lab results become structured results you can follow over time, with plain-language explanations.

  • Reminders in plain words

"Remind me every October to get a flu shot." Reminders arrive in the app and by notification.

  • A view of what may be missing

Perennial shows which records may be missing from your history and helps you request them.

  • A clinician’s look at your history

Your membership includes a review of your medical history by a licensed clinician, and setup includes an intake visit with one.

  • Room for a parent, too

Run a profile for someone you look after, such as a parent, and share it with family or a doctor. You choose who can see it.

Your own assistant

Connect Claude, ChatGPT or Grok to your records. With your permission it can read your records, labs and measurements, and with write access it can set reminders for you. Then “when was my last mammogram?” gets an answer from your own file.

It’s also in Triangle, under Settings → AI service connections → Connect.

  1. Open your assistant’s settings and find Connectors, MCP or Tools.

  2. Add a new connection and paste the address.

  3. Sign in to Triangle when it asks.

  4. Choose what it can read or change and which profile to share, then Accept.

Other assistants that support MCP (Model Context Protocol) connect the same way.

Ask it, for example

  1. When was my last colonoscopy, and what did the report suggest?
  2. Make a table of every screening in my records, with the date and the result.
  3. Remind me every October to get a flu shot.Needs write access

Said plainly: once you connect an outside assistant, what it reads is handled under that assistant’s own privacy policy, which Triangle doesn’t control. Connect only assistants you trust. It works only with what’s in Triangle and can’t change anything at a hospital or clinic.

Disconnect any time in Triangle under Settings → AI service connections, and access stops immediately. If you run a profile for a parent, you can connect your own Claude or ChatGPT to it and choose what it can see. Your parent can still connect their own.

Questions

Plain answers

What does it cost?

Perennial starts with a 7-day free trial. A card is needed to start, and if you cancel before day 7 you're not charged. After the trial it's $9 a month, and you can cancel any month; your records stay yours. The membership covers Triangle requesting your records from hospitals and clinics through the national networks and keeping them updated, chat anytime with the assistant that answers from your own records, a review of your medical history by a licensed clinician, connecting your wearables, and uploading anything else you have.

Will Perennial tell me which screenings I need?

It shows what your records say you've had, when, and what the reports found. Which screenings you need depends on your age, history and risk, and that's a conversation with your doctor. Perennial helps you arrive with the facts, and reminds you on the dates you choose.

Will it find records from all my doctors?

Triangle requests your records through the national health-record networks, Carequality and TEFCA. Not every practice, imaging center or pharmacy can be reached that way. Whatever doesn't come through, you can upload: PDFs, photos of paper records, discs and faxes. Perennial shows what may be missing and helps you request it.

What does setup ask of me?

After you start your trial, you confirm your details and verify your identity, choose who can see your profile, sign the consent forms and book an intake visit with a clinician. Then Triangle requests your records, and you can upload anything else you have.

Can it order a screening or diagnose anything?

No. Your doctor orders screenings, and you book them the way you normally would. Triangle does not diagnose, treat or prescribe, and it never replaces your care team. It helps you understand your records and prepare questions for your doctor.

Who can see my records?

Only you and the people you choose. You decide who can see your profile and who can help manage it. Your data is never sold or shared for advertising, everything is encrypted, and your records are yours to delete.
